This site is Polaris 1.10.0. It is no longer maintained and should not be used for new integration work. App partners should use the Nexus EMR IG above.

Background

Polaris is a FHIR-based specification for harmonizing clinical data across heterogeneous EMR systems. This repository defines and maintains the official Polaris FHIR profiles, value sets, extensions, examples, and supporting documentation.

Project Overview

Polaris enables standardized representation of primary care EMR data from diverse EMR products. Each EMR system maps its internal data to the Polaris FHIR specification.

This specification is:

• Application-agnostic – No vendor-specific assumptions
• CA-Core+ compliant – Built on the Canadian FHIR baseline
• Profile-light – Only what's needed, nothing extra
• Modular – Profiles evolve in stages: alpha → review → release
